Any employee who received a Confirmation Statement advising they Stored but did not Submit their elections and therefore didn't complete Open Enrollment has received a Confirmation Statement requiring a "Yes" or "No" response as well as their signature. Those employees must review, affirm and sign off on their elections and return the completed Confirmation Statement to SEGIP no later than Dec. 15, 2009 .
Most employees are NOT required to respond to the mailing. Only potential errors should be reported to the SEGIP for investigation.
Employees should review their Confirmation Statement for errors. Employees who believe an error has occurred should indicate the error by crossing out and replacing the information they believe is incorrect with the information they believe to be correct. Those employees should then sign and return their disputed statement to SEGIP by mail or by fax. The instructions are printed on the Confirmation Statement.
Employees must submit the potential errors in writing.
All Confirmation Statements for review must be received in SEGIP’s/MMB office by December 15, 2009.
